This registration form is for Westside Parents/Students ONLY.

If you are a Northside Parent or Student, then please navigate away from this form and click on the Northside Campus "Unite Youth Gathering" event graphic.

Please remember. If your student fulfills their commitment in joining us for this event, then they will automatically receive a free ticket to our KingPins Bowling event on Sunday, Sept. 25th at 2pm.

Let us know if your student will be able to join us for that or not.

*TRANSPORTATION IS PROVIDED FOR THE UNITE YOUTH GATHERING EVENT*

We are providing Transportation for this event. One of the church vans will take the volunteers and students on over to the UNITE GATHERING at AJC (A Jesus Church) located at:

10500 SW Nimbus Ave T, Portland, OR 97223

Please drop your student off at MTO WESTSIDE at 5:45pm if you elect to utilize the transportation service.
